I have several credit cards with a total of about $15.000 credit limit in credit limit that I have built within past 3 years. I applied for City bank credit card recently and received a letter in mail that they can not process my application I guess because my name on the offer that they have sent me is different than my name (one letter is missing). They want me to send them the copy of last utility bill as a proof of my address so that they could process my application. In case my name does not appear on the utility bill, which is the case, I should send them a copy of both sides of my driver's license as well as copy of my social security number. I am concerned that somebody could take an advantage of all submitted informations. Is this procedure usual?

yes, it is usual!. If they will grant you a card they will need to be sure who you are.
