At the foot of the app for Blue form America is an insurance section on life and liability. Any extra cost for this?

Yes, if you click on terms and conditions when you are in the Blue application form page, down the bottom the rate info is available. Here is an excerpt:

"Coverage and Monthly Costs per $100 of Outstanding Balance: The complete Credit Protection Plan costs: AL 81.7¢, AR 93.5¢, AZ 81.4¢, CA 80.9¢, CT 30.4¢, CO 50.1¢" etc.....
