I was sent a form pre-approved for a visa card. However I misplaced it and the response date is 6/14/01. I need a number to contact so I may speak with a person about my account.

You'll need to contact the bank that sent you the offer. Do you remember the name of the bank? If so, they most likely have an 800# that you can call to speak with someone.
